I hereby give permission for my child to participate in the summer camp sponsored by Rock Prairie Baptist Church. I/We, in clicking "agree", have legal custody of the above named child, and I am aware of and assume all risks and hazards incidental to such participation, and I do hereby waive, release and agree to hold harmless Rock Prairie Church, as well as its pastors, employees, agents and volunteer workers, from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my/our child's involvement. I also certify that my child has a registration form on file with the church, and that the information is correct.
